Services

SCR, Inc. is approved to provide Service Coordination to participants in the Office of Long Term Living (OLTL) and PA Department of Aging (PDA) Waivers/Programs in all Pennsylvania counties. These services assist participants in gaining access to needed Waiver/Program services and other state plan services, as well as medical, social, educational and other services regardless of the funding source. Service coordination is working with and at the direction of the participant whenever possible to identify, coordinate, and facilitate Waiver services. 

Primary Service Coordinator activities:

  • Verifying waiver/program eligibility.
  • Collaborating with the participant to meet their needs & goals.
  • Educating the participant on available services & self-direction.
  • Developing an Individual Service Plan (ISP) by the direction of the participant.
  • Monitoring the participant’s health & safety.
  • Monitoring the services for the participant.

 

Waiver/Program services may include:

  • Accessibility Adaptations, Equipment, Technology and Medical Supplies.
  • Community Integration Services.
  • Financial Management Services.
  • Home Health Care.
  • Non-Medial Transportation.
  • Personal Assistance Services.
  • Personal Emergency Response System.
  • Respite Services.
  • Physical and Occupational Therapy.

 

The OLTL Waiver/Programs are designed to keep people out of institutions and allow them to live in their communities. SCR, Inc. is currently approved to provide Service Coordination under the OLTL Waiver/Programs outlined below. Participant Information Packets are provided to all participants receiving services from the OLTL Waiver/Programs. These packets provide information to the participant about their rights and responsibilities, free choice of providers, the role of the service coordinator, how to file complaints and request a hearing or appeal, how to detect and report Medicaid fraud and abuse, who to contact on a variety of issues and where to find local resources.

Attendant Care Waiver (ACW) – For Pennsylvania residents between the ages of 18-59, with a medically determinable physical impairment that is expected to last of a continuous period of not less than twelve (12) calendar months or that may result in death. Eligible participants must meet the level of care needs for a Skilled Nursing Facility and financial requirements as determined by their local County Assistance Office.

Act 150 Program (Act 150) – For Pennsylvania residents between the ages of 18-59, with a medically determinable physical impairment that is expected to last of a continuous period of not less than twelve (12) calendar months or that may result in death. No financial requirements, but a co-payment may be assessed. The co-payment is based on your income and will not be more than the total costs of services.

Independence Waiver (IW) – For Pennsylvania residents between the ages of 18-59, with a physical disability, but not with mental retardation or have a major mental disorder as a primary diagnosis, or who is ventilator dependent. Eligible participants must meet the level of care needs for a Skilled Nursing Facility and financial requirements as determined by their local County Assistance Office. In addition, the disability must result in substantial functional limitations in three or more of the following major life activities: self-care, understanding and use of language, learning, mobility, self-direction and/or capacity for independent living.

OBRA Waiver (OW) – For Pennsylvania residents between the ages of 18-59, with a severe developmental physical disability requiring an Intermediate Care Facility/Other Related Conditions (ICF/ORC) level of care. Eligible participants must meet financial requirements as determined by their local County Assistance Office. In addition, the disability must result in substantial functional limitations in three or more of the following major life activities: self-care, understanding and use of language, learning, mobility, self-direction and/or capacity for independent living.

CommCare Waiver (CC) – For Pennsylvania residents age 21 or older, with a medically determinable diagnosis of traumatic brain injury (TBI). Eligible participants must meet the level of care needs for a Skilled Nursing Facility and financial requirements as determined by their local County Assistance Office.

Pennsylvania Department of Aging Waiver (PDA) – For Pennsylvania residents age 60 or older, who meet the level of care needs for a Skilled Nursing Facility. Financial requirements are determined by local County Assistance Offices.
