Who we are
Service Coordination Resources, Inc. (SCR) is a non-profit organization providing case management services to adults with disabilities currently receiving at home care through Pennsylvania’s Home and Community Based Service (HCBS) programs.
HCBS Waivers/Programs offer an alternative to nursing home placement and allow people with disabilities to live independently in their homes. SCR’s Service Coordinators work with individuals and their families to develop service plans tailored to their needs and goals.
SCR was created as a separate, standalone agency from Community Resources for Independence (CRI) due to program regulation changes in Pennsylvania’s HCBS programs. SCR started providing services on February 1st, 2013, bringing over 20 years of case management experience from CRI.
The transition was accomplished by transferring staff and establishing processes and procedures to continue the quality services that participants receiving case management services had come to expect.
