What we do
Service coordinators are case managers assigned to work with participants in Pennsylvania Waiver¹ programs. Service coordinators work closely with participants and their families to identify what services the individual may need to be able to live independently in their community.
Once the service plan is implemented, Service Coordinators monitor the services to assure the participant’s health and welfare. Participants may rely on their Service Coordinators to connect them to services and goods regardless of the funding source. It is a Service Coordinator’s job to educate individuals about services available to them.
¹The name waiver comes from the fact that the federal government "waives" medical assistance rules for institutional care in order for Pennsylvania to use the funds for Home and Community Based Services (HCBS). HCBS are also known as Waiver Funded Services or Waiver Programs.
